The training of IMGs is highly concentrated among the world's highest producing medical schools. In 1981, 25% of all IMGs trained in just ten medical schools, a third trained in just 20 medical schools. In some countries, such as India, South Korea, and the former USSR, IMGs attend a number of different medical schools. In other countries, such as Iraq, Cuba, Lebanon, Grenada, the majority of IMGs train at only one medical school.

In 1981 the top five IMG producing schools were: Santo Thomas (Philippines), Instituto Superior Habana (Cuba), U. Auto. de Guadalajara (Mexico), Far Eastern U (Philippines), and the Tehran U. Faculty of Medicine (Iran). Those five schools combined produced 15,854 IMGs. This compares to the five highest producing US medical schools (U. of Illinois, Jefferson Medical College, Indiana U., U. of Michigan, and the U. of Minnesota), which produced 35,755 physicians practicing in the US in 1981.
In 1986 Santo Tomas (Philippines), U. de Guadalajara (Mexico), Far Eastern U. (Philippines), and the Tehran U (Iran). Faculty of Medicine continued to send the most IMGs to the United States.Those five schools produced 18,288 IMGs practicing in the US in 1986. This compares to the highest producing US medical schools (U of Illinois, Indiana U, U of Michigan, Jefferson Medical College, and U of Minnesota), which had produced 39,333 practicing physicians by 1986.
In 1991 little changed in terms of which schools sent the most IMGs to the United States. Santo Tomas (Philippines), U. de Guadalajara (Mexico), Far Eastern U. (Philippines), Inst. Sup. de Habana (Cuba), and Tehran U. (Iran) continued to top the list. These five schools combined produced 19,335 practicing IMGs. That compares to the top five producing schools in the US (U. of Illinois, (Indiana U, U of Minnesota, U of Michigan, and Jefferson Medical College) which had produced 42,906 practicing physicians by 1991.
In 1996 Santo Tomas, U. Auto de Guadalajara, and Far Eastern U, continued to send the most IMGs to the US. The University of the Philippines and Grant Medical College (India) were also in the top five that year. The Inst. Sup. de Habana (Cuba), and the Tehran U. (Iran) dropped on the list of the top IMG producing schools to 12th and 10th, respectively. In 1996 20,178 IMGs had trained in the top five schools, which compares to 44,788 practicing physicians produced by the five highest-producing schools in the US .
In 2001, Santo Tomas (Philippines) and U. Auto de Guadalajara (Mexico) remained the top two IMG producing schools in the world. St. George's (Grenada) and Ross University in Dominica made the top twenty list for the first time at 3rd and 6th respectively. As in previous years, Far Eastern U in the Philippines (4th) and Dow Medical College in India (5th) continued to graduate large numbers of IMGs. In 2001 the top five schools had produced 22,037 practicing IMGs, which compares to 47,932 practicing physicians produced by the top five schools in the US.
