Swedish Family Medicine has three training sites where residents see their continuity clinic patients. 

Although each experience is different at each site, every resident at Swedish Family Medicine

has the same curriculum and rotation experiences.


Downtown Family Medicine (DFM)

 

Downtown Family Medicine (DFM) is a joint project of Swedish Family Medicine and Public-Health, Seattle & King County. The clinic is part of the safety net serving Seattle's poor and underserved population.

 

  • Continuity clinic for a total of 9 residents, 3 per year.

  • The clinic is located within the Downtown Public Health Service Center in the Belltown neighborhood of Seattle, less than 10 minutes from the Swedish First hill campus.

  • The patient population at DFM is multi-cultural and multi-lingual; including refugees and immigrants from Southeast Asia, Central and South America, Africa and Eastern Europe, as well as homeless people, unemployed persons and working poor individuals. Half of the patients have no medical insurance, and the majority of the other half has state insurance.

  • DFM operates a weekly procedure clinic where residents perform vasectomies, circumcisions, OB ultrasound, colposcopies, and minor surgical procedures under the guidance of a faculty member.

  • Residents benefit from teaching and weekly co-visits with our faculty psychiatrist and psychologist.

  • Many patients at DFM have multiple chronic illnesses. Residents also see newborns and children, patients with mental illness, as well as low and moderate risk prenatal patients.

  • Residents at DFM work with other Public Health employees, including nurse practitioners, public health nurses, and medical interpreters. The Downtown Public Health site employs dentists, travel nurses, refugee screening nurses, pharmacists and WIC clinic staff who work together to build a multi-disciplinary collaborative model of patient care.

 

Swedish Ballard Community Health

SCH is the residency's newest clinic, having opened in 2009. Located in the Ballard Swedish Hospital, it is based on the medical home model and is a pilot project for Swedish.  We have MyChart (secure e-mail for patients), phone visits, and plan to incorporate group visits for patients with chronic disease.

  • Six residents total, two per year.
  • One preceptor available in clinic (for a maximum of four residents).
  • Providers are organized into two teams comprised of 1 intern, R2, R3, nurse, and faculty member.
  • 2 full-time nurses and 1 part-time nurse.
  • 60 minute new patient visits, 30 minute follow-up visits, scheduled telephone visits.
  • Jonathan Wulf, PhD (Therapist) sees patients on Thursdays
  • Jennifer Vincent, MD (Adult and Child Psychiatry) is available for consultation and co-visits with residents.
  • Diabetes education in clinic currently once a month, increasing based on high demand.
  • Patient population: Socio-economically diverse. Some Latino, African, SE Asian patients in addition to Caucasian. Our patients are 40% self pay (many self-employed, small business or unemployed), 22% Molina, 14% Medicaid, 10% Medicare and 14% Financial Assistance. 10% of our patients have been referred from the Ballard emergency department.
  • In office lab: U/A, lipid panel, blood glucose, HgbA1c.
  • Procedures include: EKG, NST, colposcopy, IUD insertion, anoscopy, hemorrhoid banding, circumcision, surgitron available for skin tag/mole removal. Some procedures such as LEEPs need to be done at First Hill.

Swedish Family Medicine First Hill
 

Swedish Family Medicine First Hill (SFM) is located on the First Hill campus and serves as the continuity clinic for 18 residents (6 residents per year) and the majority of the residency's faculty.

·        SFM serves a diverse multicultural and multilingual patient population, with patients from all races, ages and demographic backgrounds. Approximately 50% of patients are insured by Medicare or Medicaid. Uninsured patients are seen through Swedish charity care.

·         We have about 20,000 patient visits per year, providing training in full- spectrum family medicine including chronic disease management, pediatrics, obstetrics, and geriatrics.

·        We utilize registered nurses and medical assistants experienced in ambulatory care and residency teaching for clinical support.  SFM has a laboratory and the ability to perform a variety of diagnostic procedures such as EKGs, NSTs, etc.

·         Residents participate in specialty clinics including procedure, ultrasound, gynecology, surgery, and psychiatry clinics.

·         Residents benefit from working with a full-time social worker and part-time pharmacists.

·      The residency program’s clinical psychologist provides teaching, patient care, and co-visits for residents at SFM.

·     Consultations from regularly scheduled specialists in orthopedics, pediatrics, podiatry, and psychiatry also provide teaching opportunities.

·      SFM utilizes EPIC EMR, and every computer in the clinic has easy access to numerous medical resources.
