After confirming availability of dates by consulting to the online calendar, students should then apply through the UW Medical School Clinical Student Elective Program to determine if they qualify for the clerkship.

Qualified students may then submit the following application materials directly to the Program Administrator:

• 2 letters of recommendation
• Personal statement including:

1. Description of your goals for an Advanced Family Medicine Rotation and how they believe these goals may be met by a rotation at our site.
2. Description of your interest in working with underserved populations
3. Description of your areas of special interest in Family Medicine.
