Swedish Community Health Medical Home – Ballard

Residents will have regularly assigned office hours at Swedish Community Health. All schedules are posted on amion.com. Clinic office hours preempt all other resident duties, except OB/deliveries or hospital emergencies. Residents must notify the clinic chief resident immediately of such events.

EPIC is the electronic health records system utilized by all of the Swedish Medical Centers and outpatient clinics, including Swedish Community Health.  Complete documentation of office visits and all communication with patients is essential.

Clinic Team

The Swedish Community Health team consists of 6 residents, two RN's, a clinic coordinator, a patient services representative and two clinical faculty FTE's.

Patient Panels

Clinic patients are assigned to the residents by families. When care for a given family member must be delegated to another resident because the primary resident is unavailable, coverage is to be provided primarily by another team resident. Follow through to resolution of the illness is done by that physician or by the primary physician when they are available.

Clinic Hours

Swedish Community Health is open 9:00 AM until 5:00 PM Monday, Wednesday through Friday, and 8:30 AM to 5:30 PM Tuesday.

Clinic Schedule Changes

If it is necessary to make a change in your clinic schedule, such changes must be made in advance. Any change of clinic schedules must be approved by the Swedish Community Health Chief Resident.

Clinic schedules are made 1-2 months in advance. Residents are responsible for reviewing their clinic and call schedules and notifying the Clinic Chief of any conflicts.

Hemoglobin
Urinalysis
Pregnancy Test
Lipid panel
Fingerstick glucose
A1C
Wet mount
KOH preparation for fungi
Ferning test on cervical mucus
EKG
Peak Flow
NST's

Most other blood tests may be drawn in the Swedish Hospital - Ballard lab.

Behavioral Scientist Faculty

Jonathan Wulf, Psy.D., is a clinical psychologist on the faculty. His primary role is to teach residents to identify and intervene in psychosocial issues of patients. Dr. Wulf is available at the clinic Thursday afternoons for consultation and to see patients. He is also available as a co-therapist for individuals, couples, and families seen with their primary resident. Additionally, he is available for crisis intervention and referral to appropriate community agencies.

Chart Review

Faculty review all charts with care suggestions, allowing residents to learn from different preceptors' suggestions.

Chart Summary

Summary dictations will be done on complicated patients when R-3s finish the program.

Teaching in the Clinic

Faculty supervision is always available in the clinic. Preceptors are available at all times to answer questions, to aid with patient diagnoses or procedures and to help with all care issues. Behavioral science faculty is available one afternoon each week. Colposcopies, OB ultrasound and other procedures can be scheduled during regular clinic hours. Mammogram is available on site through the hospital.
