  A.  "Residents will be proficient in the diagnosis and management of acute and chronic disease and the principles of preventive medicine."

        2.  Recognize patients requiring hospitalization in an outpatient setting

   B.  Chronic Disease  
  1.  Diagnose and longitudinally manage chronic disease to optimize function and independence.  
    2.  Understand the use of organizational tools such as flow sheets, templates using practice guidelines, EMR, population databases, disease management tracking and recall strategies.  
    3.  Understand and utilize the multidisciplinary approach to manage chronic disease.
