Newborn Care Curriculum
The goal of the rotation is to improve
resident's and Fellows understanding and comfort with newborn care. Our
objective is to provide opportunities to experience and study the care of
healthy newborns and sick or premature infants.
Important information: please review two
weeks before starting this rotation.
There are two main elements to the rotation, and
four smaller elements. The two main elements are rounding in the Special Care
Nursery to learn the care of sick infants, and carrying the resuscitation pager
to learn to resuscitate and admit newborns. The four smaller elements are:
Learning the routine Newborn Exam with Dr. Uman; Circumcision Clinic with Dr.
Nielson; Lactation consultation; and Nutrition consultation.
Requirements:

NALS certification
required prior to start of rotation.

This is a morning
rotation, so continuity clinics must be scheduled in the afternoon.

Special Care
Nursery
All mornings will begin at 8:00 AM with rounds
with the Neonatologist in the Special Care Nursery. Usually this is with Dr.
Paula Raschko, but can vary. She will help you chose three sick infants to
round on each morning. Hand-write a note on the chart, and hand-write orders.
Babies that you admit while you are carrying the resuscitation pager are good
patients to follow.
Newborn Resuscitation
The entire time you are in the hospital you will carry the Newborn Resuscitation
pager, and respond to all calls. This takes priority over all other activities
except your own continuity OB. Pagers are kept in the Gumby Lounge. When you
start the rotation, obtain the pager in Gumby, and leave it there when the
rotation is completed. Place a call to the operator when starting the rotation
to test the pager. This pager should be set to receive all three levels of
pages. Wear this pager anytime you are physically on campus during the two
weeks, and document all procedures and resuscitations.
Additional
Sessions:
Circumcision Clinic
Tuesdays at 10:00, attend the circumcision clinic with Dr. Nielson. Contact him
prior to the rotation to tell him you will be there. He will have you observe
several, and do several under supervision. He also has slides that allow you to
learn when to not perform circumcisions and potential problems and pitfalls.
Newborn Exam
Dr. Uman is available on Tuesday, Wednesday and Thursday to teach the Newborn
Exam. You must contact him by phone or email (see contact information below)
and arrange at least two teaching sessions with him. Infants on the Special
care nursery will be examined. The goals of this aspect of the rotation
are:

Learn how to perform a
systematic exam of newborns

Learn the
evidence for each element of the exam

Know the normal variants
-
Know the abnormalities
screened for by the exam, and how to proceed if one is discovered

Session with Joy Coale. Joy is the
nutritionist for the SCN and has very useful information regarding
newborn nutrition. Please contact her early in your rotation to arrange

Attendance at a single session is required.
Read on Enteral feeding of Newborns.

Morning with the lactation consultant. The
lactation consultants visit all of the newborns at Swedish and rounding
with them to see common breast-feeding questions and answers. Ms. Moore
needs 48 hours arrange the schedule. Call on your first day and set this

Newborn Exam with
Dr. Howard Uman. Dr. Uman is usually available to teach this skill on
Tuesday, Wednesday and Thursday at 8:30. He will often have a 1st
year resident as well, and the Second Year resident should assist in the
teaching of the exam. A minimum of two teaching session need to be documented. The Newborn Exam module on the Online
Curriculum is good to review prior to the first exam.

Carry resuscitation pager entire time that you are in
house. (Pager will be clipped to the patient board in L&D.) Attend all
resuscitations and round on all babies subsequently admitted to the Step
Down Unit. Document all resuscitations and intubations. |
Interpersonal and Communications Skills
Demonstrate clear communication with patient regarding their medical
issues in a culturally sensitive manner
Demonstrate team oriented communication and foster working
relationships with other member of the care team or specialist colleagues
Professionalism
Demonstrate a high degree of professional responsibility in managing
patients with
Neonatal Outpatient Pediatrics-related problems by referral coordination, direct management
and follow up.
Demonstrate professional demeanor, attire and appearance in all
settings.

Demonstrate understanding of the role of the PCP in the immediate
and after-care of patients with
Neonatal Outpatient Pediatrics-related diagnoses.
