OB GUIDE FOR R1’S

Daily schedule

 

Prerounding:  You will be assigned to one or two perinatal patients.  You need to preround on these prior to meeting with fellows and other R1s at 7:50am.  You also should round on any postpartum women you have delivered prior to 7:50.

 

7:50 L&D rounds: usually at “the board” on L & D (5E) and involves the post-call intern presenting / signing out to the on-call intern and the rest of the group.

 

8:00 - 8:30 didactics: R1s, fellows, UW R3 for teaching by somebody.  R1s usually teach one of these sessions each block.  Very informal, often in cafeteria.

 

8:30 - 9:15 rounds with perinatologist.  Brief presentation of the patient(s) you are following.  Usually not much changes with these patients.

 

Rest of day: post-call intern goes home.  On-call intern gets busy.  Clinic if scheduled.  Everyone else has no further formal responsibilities. 

 

 

Exceptions

  

Thursdays: 4th Thursday each month is M & M at 7:30 AM in the Locke room, 2nd floor;

 

Weekends: split ALL pts between on and off going senior/fellow and interns. Meaning POSTPARTUMS as well.  Weekend interns should see fellow (not on call) interns post-partums.

  

R1 responsibilities

 

1. Be a sponge! Learn as much as possible!  Ask nurses/fellows/attendings lots of questions (maybe not in front of laboring patients).

 

2. Follow 1-2 antepartum patients; pre-round and leave a note on each pt PRIOR to rounds with perinatologist at 8:30 AM; present your pt at rounds, stating the basics first: "Ms X is a G2 P1 at 30 2/7 wks by first trimester US admitted for PROM, etc"

 

3. Round on all your post-partum pts PRIOR to 0750 AM rounds (this helps keep attendings happy, and aware that we are involved!)

 

4. Help out the team: With post-partums, ante-partums if your load is light.

 

5. While on call: Good idea to meet the L and D charge nurse and introduce yourself.  Nurses are key to L& D ! be diplomatic as much as possible!

 

   •     Get sign-out at change of shift from the off going intern by meeting each laboring pt with him/her, and the nurse (key!).

   •     Leave a note at least every 2 hours on pts admitted to L& D- medically/ legally a necessity, and it shows the attending YOU ARE INVOLVED!

   •     Call the fellow/R2 for any triage pts, and any concerns/ questions, anything you feel uncomfortable with, at ANY hour. Also, if you are just swamped, we can sometimes help out, ie see triage pts or laboring ladies you can’t get to right away.

   •     Roll the dates forward after midnight on OB signout list (all the antepartum pts).

 

6. Be prepared for teaching at 8:00 AM by reading ahead. The schedule for upcoming topics will be written in the rounds room, and participation makes this a lot more fun!

 

2. Each intern should do a BRIEF presentation on a topic of their choice (5- 10min). i.e. GBS, birth control postpartum, PTL management, shoulders, etc

 

3. VERY IMPORTANT! Let us know if something isn’t working for you! We will try and accommodate you as much as we can!
