Rotation Evaluation Form

Please print the rotation evaluation form and have it signed by the main attending for every rotation on your last day, and submit this to Sonya to be placed in your file. We are required by the RRC to have an evaluation completed for you on every single one of your rotations. On your first day of the rotation, you should discuss the learning objectives with the attending, and invite them to view the online curriculum to focus their teaching. On the last day of the rotation, you need to present them with this evaluation.  They can choose to fax or mail it in, or simply hand back to you once completed. You need to have this evaluation completed to pass the rotation.
