The Accreditation Council for
Graduate Medical Education (ACGME) mandates that "There must be a method of
documenting the procedures that are performed by the residents. Such
documentation must be maintained by the program, be available for review by the
site visitor, and be used to provide documentation for future hospital
privileges." Swedish Family Medicine Residency Program agrees that the above noted requirement shall be met by having
each Resident document said experience on a PDA handheld computer, which will be
supplied to each resident by the Program.
Swedish Family Medicine and the
Resident agree that:

The
PDA is the
exclusive property of Swedish Family Medicine Residency. Should Resident lose or damage the
PDA through acts of
negligence such that it is not useable, the Resident agrees to repair or
replace the PDA within 30 days at Resident's expense.

Swedish Family Medicine will
provide the Resident with a PDA at the beginning of the R1 year.
Instruction on how to use the PDA will be provided by Chris Vincent
and others.

Throughout residency, the
Resident will enter the required information on their PDA or online via New
Innovations. This is to
be done during each hospital-based and office-based rotation and during the
family practice center experience.

At least once a month the
Resident is required to downloaded experiential data. This information will become a part of the
permanent record maintained by Swedish Family Medicine. An experiential
report will be included as part of Resident's quarterly evaluation with
their faculty-advisor.

Swedish Family Medicine will
provide Resident with a printed summary of their entire residency experience
at the conclusion of their residency. This record may be utilized for
obtaining future hospital privileges.

Resident is required to
return the PDAto the residency administration office during the last rotation
of the third year of their residency program.
This Agreement shall become
effective immediately upon its signing and shall remain in effect until the
resident graduates from Swedish Family Medicine or until terminated by either
party.
