Course Evaluations
On the last day of your clerkship you wil receive an email asking you to evaluate your family medicine clerkship experience. Your responses are confidential and will not be shared with your site coordinators until after your grade has been received. Please also keep in mind that your grade will not be given to you (if/when requested) until the evaluation(s) are submitted.
Earlier in the year, you were sent an automated email with your login ID and password (your login ID is the same as your UW NetID). If you no longer have your password, there is an option to reset it on the login page.
To access the survey(s), please copy and paste the following link into your browser's address bar, then log in to complete your evaluations.
